Assistantship Forms

Assistantship positions will not be awarded until you have been admitted into your graduate degree program. Individual academic and non-academic departments and units are responsible for award decisions, detailing the expected duties and responsibilities, stipend amounts, and establishing a work schedule. The assistantship application must be submitted to the office where you wish to be employed.

Master's

  • Committee Request
    Should be created first semester of enrollment and kept in department; due to OGS with Graduation Checksheet. All committee members must have a graduate faculty appointment.
  • Graduate Program of Study
    Should be created first semester of enrollment and kept in department; due to OGS with Graduation Checksheet.
  • Transfer Approval Form
    Should be submitted to OGS as soon as transcript is available so courses can be reviewed and transferred.

Educational Specialist

  • Committee Request
    Should be created first semester of enrollment and kept in department; due to OGS with Graduation Checksheet. All committee members must have a graduate faculty appointment.
  • Graduate Program of Study
    Should be created first semester of enrollment and kept in department; due to OGS with Graduation Checksheet.
  • Graduate Program of Study - Attachment Sheet
    Should be attached to Graduate Program of Study form. This form details courses that will count toward the total number of credit hours required for the degree but do not appear on the Program of Study. Due to OGS with Graduation Checksheet.
  • Transfer Approval Form
    Should be submitted to OGS as soon as transcript is available so courses can be reviewed and transferred.

Doctoral

  • Committee Request
    Should be created first semester of enrollment and kept in department; due to OGS when preliminary examinations are scheduled. All committee members must have a graduate faculty appointment.
  • Graduate Program of Study
    Should be created first semester of enrollment and kept in department; due to OGS when preliminary examinations are scheduled.
  • Graduate Program of Study - Attachment Sheet
    Should be attached to Program of Study form. This form details courses that will count toward the total number of credit hours required for the degree but do not appear on the Program of Study. Due to OGS when preliminary examinations are scheduled.
  • Change to Graduate Program of Study
    Should be attached to Program of Study form and kept in department until paperwork is sent to OGS when preliminary examinations are scheduled.
  • Transfer Approval Form
    Should be submitted to OGS as soon as transcript is available so courses can be reviewed and transferred.
